Example for gray zone: Liver stiffness 12.8 kPa, corresponds to the intermediate zone 10–15 kPa for cACLD. Against the background of active alcoholic liver disease, LSM may be overestimated; correlation with biochemical activity and re-evaluation after stabilization is advisable.

VCTE, pSWE, and 2D-SWE: Threshold Transferability

The numbers in the table are most applicable to transient elastography. For point shear wave elastography and 2D-SWE, results are also expressed in kPa, but inter-device transferability is limited: different systems form the wave and ROI differently. Therefore, the protocol should specify the specific technology and not mechanically transfer VCTE thresholds to another device without local validation and manufacturer recommendations.

The Role of Multiparametric Ultrasound

WFUMB 2024 considers elastography as part of multiparametric liver ultrasound. In ALD, it is useful to simultaneously assess signs of steatosis, liver morphology, spleen, ascites, portal blood flow, and collaterals.
