Renal Resistive Index (RI): Norms and Clinical Significance
Definition and Calculation Method
The Resistive Index (RI) reflects renal vascular resistance (RVR) and is calculated using the formula: (peak systolic velocity − end diastolic velocity) / peak systolic velocity (EFSUMB Course Book, 2024). Spectral analysis of interlobar arteries allows for quantitative assessment of parenchymal perfusion; Vmax and Vmin are determined, after which RI is calculated (EFSUMB Course Book, 2020). RI and acceleration time (AT) do not require angle correction.
For optimal spectral analysis, the wall filter is set to the lowest possible values to avoid eliminating low diastolic blood flow, and the pulse repetition frequency (PRF) is set to a minimum to avoid aliasing (EFSUMB Course Book, 2024).

For adults, an RI threshold of 0.70 is an indicator of normal RVR; all values above 0.70 indicate increased vascular resistance, which is important in assessing many pathological conditions, especially parenchymal kidney diseases (EFSUMB Course Book, 2024).
Age-Related Features
In children, RI values are higher in the first 2 years of life and gradually approach adult values between 2 and 6 years. Threshold RI values should not be applied to children under 6 years (EFSUMB Course Book, 2024). RI may exceed 0.70 in some normal conditions — in newborns and children up to 6 years, as well as in elderly patients (Hagen-Ansert, 2023).
Clinical Significance
Acute Urinary Tract Obstruction
In acute obstruction, the RI of interlobar and arcuate intrarenal vessels may exceed 0.70, starting 6 hours after the acute onset and lasting up to 72 hours; RI returns to normal values after 120 hours of obstruction (Hagen-Ansert, 2023). According to EFSUMB (2024), RI usually increases within 6–48 hours after the onset of obstruction (example: RI 0.74 in the obstructed kidney versus 0.60 in the non-obstructed, dRI 0.14).
The level of obstruction affects the degree of RI increase: the indicator is higher with obstruction in the proximal ureter or distal intramural segment. The use of NSAIDs may reduce RI on the affected side, decreasing the sensitivity of Doppler in detecting obstruction (Hagen-Ansert, 2023). An elevated RI > 0.7 in obstruction is nonspecific and rarely diagnostically significant (Diagnostic Ultrasound: Abdomen & Pelvis, 2022).
Parenchymal Diseases and Nephrosclerosis
RI and AT are important in monitoring transplanted kidneys and assessing diffuse parenchymal diseases (EFSUMB Course Book, 2020). In nephrosclerosis, RI increases to 0.8 and above; a high value corresponds to rapid progression to renal failure (EFSUMB Course Book, 2020). RI elevation may also be noted in intrinsic kidney diseases, diabetes, and hypertension (Hagen-Ansert, 2023).
